Billing and Insurance Information
Thank you for choosing Family Healthcare Partners for your healthcare needs. To ensure that we provide you the best service we can, please review the following policies regarding our services:
- We accept most all types of insurance. To see a list of insurances for 2008, click here (PDF).
- For your convenience the following methods of payment are accepted:
- Cash / Cashiers Check / Money Order
- Visa, Master Card, American Express and Discover Card
- As a courtesy to you, we will bill your insurance company. If your insurance does not settle or does not accept your claim, you are responsible for payment in full.
- All co-pays, or self payment are due in full at the time of service.
- You are responsible for current insurance and relevant personal information. If your insurance or any of your personal information changes (name change, address, phone), please notify us immediately.
- We accept assignment for Medicare. You are responsible for the annual deductible and all amounts not covered by Medicare. If you have supplemental insurance, we will also bill that insurance; however, you are still responsible if the second insurance has co-pays or deductibles.
- If you are covered by Medicaid, you must have a valid card with you in order to be seen. If you do not have a valid card, payment in full is required at the time of service.
- A fee will be charged for broken appointments or appointments cancelled without a 24 hour notice. Collection fees will be assessed for returned checks and non-sufficient funds.
- We must emphasize that as medical care providers, our relationship is with you, not your insurance company. While filing insurance claims is a courtesy we extend to our patients, all charges are your responsibility from the date the services are rendered.
